Medicine in an anarchy. Cheep, High Quality Care.

I’ve been considering the numbers behind the $25 I had Joe Legal and Jose Illegal paying for membership in their Mutual Aid Society. I thought I should clarify what they get for their money, and other options that might be available in a truly free market. Assuming that the Society Joe/Jose belong to has 1000 member families it can pay out up to $25000 per year for major medical issues among those families. This may not seem like a lot but remember that this is a fully free market. The overhead in the medical field would be reduced by the massive competition available. Most doctors would work primarily with life threatening illnesses, severe trauma, and chronic conditions. Minor issues such as colds, flu, simple infections like strep throat and bronchitis could be handled with a Google search for the correct dosage of an antibiotic or other medication. Minor trauma and critical care could be handled by trained personnel without the need for a full medical degree. This would leave doctors with only the need to cover major or difficult surgery, diagnosis and treatment of unusual diseases, and research to deal with life threatening disease like cancer or chronic conditions like heart disease.

In a free market there are no restrictions on the purchase of goods or services. A person with a minor illness can use his own knowledge of medicine, the vast resources of the Internet, or a local expert for the diagnosis of his own illness. He can then determine, through one of the means previously mentioned, the correct course of treatment, the medications needed, and the dosage required. He can then go, and with or without instruction, and at his own risk, to the local pharmacy and purchase the medication he believes will be most effective in relieving his condition. If Joe Legal gets a sinus infection, and knows he has a sinus infection, either because he has had one before or from information presented on line that he believes to be trustworthy, he can go to the pharmacy and ask the pharmacist to provide the correct antibiotic. He then takes the antibiotic according to the instructions on the bottle, and soon has no sinus infection. In the stateist society we live in today he must go to a state licenced doctor, who will tell him the information he already knows and give him a prescription to buy the medication he knows he needs, then charge him $200 for telling him things he already knew. In the free market Jose Illegal faces a similar situation to Joe Legal and soon has his medication. In the stateist society Jose goes to a government run clinic and, when the bureaucracy is ready, sees a doctor who doesn’t care about Jose getting better because he gets paid anyway. He then takes his government health care card to the pharmacy and gets his medication. The government will pay the doctor and the pharmacist with money that they stole from Joe Legal and other tax payers like him.

In the realm of minor trauma, such as a broken bone, or critical care, such as appendicitis, the free market provides a large number of options not available in the stateist society. Currently EMT’s and paramedics are the first responders in a medical emergency, however the procedures they may perform are limited by law. In the free market the EMT or other first responder would have no such restrictions. They could set bones, perform a relatively simple and common surgery such as appendectomy and administer needed medication with a greater degree of freedom than is currently available. Let us suppose Joe is out one day in his yard and steps on his sons roller skate. He loses his balance, falls, and breaks his leg. In the free market he, or more likely his wife, calls the mutual aid society’s first responder line and reports that Joe has fallen and probably has a broken leg. They dispatch a paramedic who comes out, sets the leg, provides a pain killer and immobilizes the broken bone. In the event of a more severe injury the paramedic will take Joe to the hospital for further evaluation, otherwise he will advise Joe to follow up with his regular medical provider as soon as possible. The mutual aid society has a vested interest in Joe getting prompt, competent care as they will have to pay out for long term care if something is botched. Jose would live in similar circumstances. The stateist society faces all the problems brought on by a bureaucracy including poor service and slow care. Jose also takes the additional risk that he will be identified as an illegal alien and deported. In addition Joe is forced to pay for the whole process through taxes, which are ultimately collected at gunpoint.

At last we come to major medical issues. Life threatening diseases, chronic conditions, long term care, major trauma and all the other ways the human body can malfunction. This is the domain of the Doctor, by which I mean an individual who has attended a medical school and earned an advanced degree. For the most part Doctors will be engaged in research at pharmaceutical companies, hospitals, and universities. While there will be fewer Doctors there will also be much fewer patients seeking care from a full Doctor. Their time will be relatively affordable and their technique will usually be on the cutting edge of medical technology. Their skill set will lie in recognizing or defining unusual medical phenomenon and finding a cure for them. in general once a cure becomes widely accepted it will also be well known, so the diseases that requite a Doctor will decrease over time. There will also be a need for skilled surgeons, who will need every bit of anatomical knowledge possible to perform surgery, especially delicate surgery, quickly, efficiently, and safely. Again as the techniques become better understood and better known the skill needed to perform them will go down, and with it cost. Contrast this with the current system where costs rise constantly, a medical degree is needed to check the most minor of problems, and nobody but a Doctor can issue even a basic antibiotic, thus driving cost up even further.

As we can see the intervention of the state pushes prices higher across the board, while it’s removal lowers them rapidly. This is accomplished by allowing people to take responsibility for the substances that go in their own bodies, allowing first responders greater leeway in dealing with emergencies, and dedicating Doctors to research into new techniques rather than the rote application of well understood medicine. Ultimately it is the violence of the state that drives medical costs higher, as violence always drives all prices higher. With this price reduction a family can afford to cover basic medical for a low price, and have protection for any major issues that may arise for as little as $25 per month.

I received this today via E-mail from a stateist family member. I’ve cleaned up the formatting  errors common to a repeatedly forwarded email and included my reply below.

Here is an example of why hiring illegal aliens is economically distructive for America…

Joe Legal sends his children to a reputable privately owned academy for $500 per month per child. This includes lunches and supervision until Joe picks them up at the end of the day.

Jose Illegal home schools his children with his wife acting as their instructor. They hire an English tutor so the children can learn English and get a good job when they grow up.

Joe and Jose both hire a Dispute Resolution Organization to cover their homes, cars, and family, for fire, burglary, assault, and vandalism. The DRO guarantees repair/replacement of stolen or damaged property and compensation and medical coverage for injuries sustained in an assault. It will investigate and collect restitution from the responsible party. They also operate a local Fire Department to put out any fires in the area, after all they don’t want the fire to spread and damage other properties they insure. Further they maintain a small number of nuclear missiles to deter attack on their members by a government.

$100 * 12 months = $1,200 leaving both with $4,500 each.

Both give generously to charity to provide a public access school, subsidized housing and other benefits to the poor.

Both are happy and successful, have leisure time and money. While Jose is home schooling and Joe sends his children to a private school both provide a good education to their children. Jose could afford the private school by moving to a smaller apartment or having his wife take a part time job, or taking a second job himself. Both also have the option to send their children to a public access school of their choice. Neither is supporting a violent war in a foreign land, nor a massive bureaucracy that ultimately only pours money down a bottomless hole. Both are free to take any action they want, own any object they want, use any drug they want and generally go about their lives in the manner they see fit. If they aggress against another person they will be treated as criminals, and expected to make a full restitution to the injured party or the DRO used by the injured party to cover the damage.

Joe is a successful contractor, and Jose is his best employee.

As you can see from the above story the problem is not that Jose crossed and imaginary line on a map without permission, It is that a violent group of criminals and thugs use different standards for different people and say it is for their benefit.

The removal of the state and it’s “benefit programs” has left both Joe and Jose above with a happier life. Joe has money and leisure time that he did not have before and Jose no longer has to live in fear of an INS agent knocking on the door and violently forcing him and his family to leave their home. Neither has to worry about violating one of the over 10,000 federal laws or countless state laws on the books. Joe doesn’t need to have every decision he makes in business approved by some bureaucrat who knows nothing about construction. Their children will receive better education brought on by competition.

Let’s see if we can make it so Joe and Jose can live in peace.

In Lord of the Rings the One Ring was a powerful symbol of corruption. It was power of such a nature that whomever held the ring would, in the end, be corrupted and destroyed by its power. Gandalf the Grey refused to take up the ring, knowing that even in an effort to use the ring for good, his actions would be turned to evil. Frodo Baggins took up the ring with the avowed intent to destroy it, when it came to the end he could not bring himself to throw the power away. The creature Gollum lost his soul and, at the last, his life to his greed for the power of the ring. Mankind now lives with such a power. A power of such evil it cannot be turned to goodness. A power that even those who seek its end are unable to lay it down. A power that causes the deaths of countless numbers of people both innocent and guilty. The U.S. Government, at least according to its founding documents, derives its powers from the consent of the governed, however it never asks for that consent. In order to form a more perfect union, it fought a 4 year civil war against a group of people who no longer consented to be governed. Establishing justice seems to be more about putting people who harm nobody but themselves by using certain substances the government doesn’t approve of in prisons that are little more than government sanctioned rape rooms. The insurance of domestic tranquility is police attacking non-violent protesters and dragging them to prisons. The common defense is aggressive war. The general welfare is robbing the productive to provide to the lazy, and the Blessings of Liberty are lost in tens of thousands of pages of rules and regulations on all manner of subjects from crossing imaginary lines on the ground to the height of the grass on your lawn. While the power of government was established with high ideals and good intentions, like the One Ring, it has corrupted its wielders and twisted their actions to evil.

In 2008 a strange thing happened. A national campain emerged arround a relitively unknown member of congress. Millions of dolars were raised to support him. His message resonated with millions. After a long campaign in the primaries, he…

lost. Ron Paul never even came close to the nomination.

Why? His message was good. He offered freedom. He promised to shrink the government. He said he wanted a balanced budget, to end the war in Iraq and lower taxes. So why didn’t this result in victory? It’s simple. He wanted to end subsidies, and lost the vote of those subsidized. He wanted free trade, and lost the votes of the unions. He threatened government jobs and lost the votes of its employees. He tried to use the ring to destroy the itself, and the ring turned on him. There was no conspiracy, it was just individuals looking for what they saw as their own best interests. Even had he ascended to the oval office, the 535 members of congress would surely have prevented his proposals from changing even one law. Unless and until he worked for the goals of government he could not truly wield it’s power. In the end, the power of the government would and did prevent him from laying down even one small part of its power. The power of the ring cannot be used for good.

In the 20th century, over 20 million people were killed by their own government. They were killed because of their race, or their tribe, or their religion or just because they disagreed with those who were in power. Concentration camps, Gulags, Internment camps, and medical experiments, many methods have been used by various governments to kill, maim, and destroy their own people. From the cold steepes of Siberia to the hot climate of Tuskegee, from Auschwitz to the Japanese internment in the US, From Abu Ghraib to Guantanamo Bay, the governments of the world have always had somewhere to put people they don’t like. Tolkein had one advantage that mankind lacks. His ring was a thing, a material object. Government is an idea, and ideas are VERY hard to destroy. To this day there are Nazis, Comunists, Monarchists and Dictators despite the fact that all these ideas have in time been discredited. Ideas must be defeated one person at a time. I am one of those people. I have lain down my part of the ring. I hope that in time, you will too.
